Page 4, line 42 strike "is" insert "shall be placed in an early ballot affidavit envelope and will be"; strike ", and" insert "if the ballot affidavit is signed and dated by an early voting election clerk stating that “I affirm that the voter provided identification in compliance with section 16-579, subsection A, paragraph 1, Arizona Revised Statutes.” and includes an identification number unique to that clerk. If the early ballot affidavit includes an affidavit as prescribed by section 16-547, the voter’s signature, the date signed, the election clerk’s signature and the election clerk’s unique identification number,"
Line 44, after the period insert "If any portion of the affidavit envelope is blank or if the date or clerk identification number is illegible, the signature verification prescribed by section 16-550 is required. The ballot affidavit envelope used at early voting locations shall be of a different color than those used for early ballots mailed to voters in order to be visually distinguishable from ballot affidavit envelopes that require signature verification, and early voting location ballot affidavit envelopes shall not be used for any purpose except for ballots voted at an early voting location after showing identification that complies with paragraph 1 of this subsection. The election clerk’s unique identification number must be traceable to a specific clerk and be able to be cross-referenced against a master schedule of clerks that can be matched, if needed, against the early voting register or e-pollbook to confirm that the voter checked in at the same early voting location where the clerk worked on the date listed on the affidavit."
