Page 15, strike lines 9 through 13, insert:
"14. If the charter school contracts with a charter management organization, a written statement from the charter school identifying the charter management organization and the name of the person or entity that has ownership of the charter management organization and describing the services provided to the charter school or charter holder and the total aggregate amount the charter school or charter holder spends for the charter management organization's services. The statement shall include an itemized list of services the charter management organization offers to the charter school or charter holder, including the cost for each service."
