FLOOR AMENDMENT EXPLANATION

 

1.    Makes a onetime appropriation of $2,500,000 in FY 2020, rather than an annual appropriation of $2,500,000 in the next three FYs.

2.    Requires the nonprofit organization to submit a quarterly report of provided services and referrals.

3.    Outlines additional information that must be included in the report.

4.    Makes conforming changes.

After line 24, insert:

"C.  The nonprofit organization shall submit to the department of health services on a form prescribed by the department a quarterly report of the services and referrals provided by the nonprofit organization. The report shall include the following information:

1.  The number of clients served, either by referral or direct services.

2.  The number of direct services provided and referrals made.

3.  The number of women referred for medical services or medical care.

4.  The number of women who received prenatal care.

5.  The number of women referred for prenatal care.

6.  The number of women who received nutrition services.

7.  The number of women referred for nutrition services.

8.  The number of parents who received adoption services.

9.  The number of individuals referred for adoption services.

10.  The number of individuals who received educational and employment services.

11.  The number of individuals referred for educational and employment services.

12.  The number of individuals who received parenting education and support services.

13.  The number of individuals referred for parenting education and support services.

14.  The number of individuals who received referrals to programs through the department of health services, the department of economic security and the Arizona health care cost containment system and other public programs, with a breakdown by program.

D.  The report submitted pursuant to subsection C of this section shall include aggregate client demographic information, including age, marital status, county of residence, ethnicity, sex and number of current children. The report may not identify any individual client by name or include any other personally identifying information or an identifier that would make it possible to identify a client, in any manner or under any circumstances."
